Home Care Service (House Visit)

Before Choosing Home Care Service

Please read the following important information carefully:

1. Home care service is meant only for patients who are

Bedridden or bed-bound
Severely mobility impaired
Unable to travel by car or ambulance
Medically not advisable to travel
Undergoing chemotherapy/radiotherapy
Prone to infections or medically vulnerable

2. Initial consultation is mandatory

The patient’s family member/attender/caregiver must visit the clinic with all medical reports and documents.

3. Before consultation

Fill the required form (Medical History Form)
Book an appointment through our website

4. Consultation with the Neuro Physiotherapist will include

Medical history review
Screening and discussion
Clarification regarding suitability for home care service

5. Assessment charges

Detailed assessment charges will be explained during the consultation.

6. Assessment process will begin only after

Consent form signing
Fees payment confirmation

7. Assessment date and time

Assessment date and time will be communicated in advance by our office.

8. Assessment report

Assessment report will be shared within approximately 48 hours after completion of assessment.

9. Discussion session

Based on the assessment report, the patient and family members can decide regarding continuation of rehabilitation services.

Home care services are usually short-term and not intended for prolonged care. Transitioning to clinic-based rehabilitation is often important for better recovery, access to equipment, structured rehabilitation, and long-term improvement.
